Please fill in the below form for ALL of your children. You can designate up to 4 adults who can pick up your children (including yourself and your spouse).
Once the school has this information, we will create a badge for each adult with permission to pick up the children. As part of our security protocols, each adult will have to show the badge to be able to pick up children during dismissal. Badges will also be required for parents when they are volunteering during the day.
